This is so funny – the graphic that is intended from this quote. Not only is it funny, but it describes the element of being tactful. The allusion to stepping on someone’s toes EVEN though he/she is wearing scruff-free shoes. In other words, you can step on someone’s toes – get to the core of the person/give an opinion; yet manage not to ruffle his feathers [his exterior/shoes/his attitude].

It is an art form.
